False PHD is a special passive item in the video game The Binding of Isaac: Repentance. It is a risky but powerful item that changes how pills work for your character during a run.

Identifies all pills: Like the regular PHD item, you can see what every pill does before you swallow it.

Gives a Black Heart: You instantly get one Black Heart when you first pick up the item.

Turns pills negative: It changes most good pills into bad or neutral pills.

Stat downs give damage: Whenever you take a pill that lowers a stat (like Luck Down or Range Down), you also get a permanent Damage Up boost!

Other bad pills give health: If you take a bad pill that does not lower your stats (like Amnesia or Paralysis), it drops a Black Heart on the floor. Why It Is Good

Many players like False PHD because Damage is the most important stat in the game. You can intentionally lower less important stats, like Shot Speed or Range, to make your character hit much harder. It is also an amazing tool for staying alive because you can generate lots of extra Black Hearts throughout your run.
